论 著 基 金 项 目 ： 浙 江 省 科 技 计 划 项 目 资 助 （ 991104209）作 者 单 位 ： 310031 杭 州 ， 浙 江 大 学 医 学 院 社 会 医 学 教 研 室 （ 李 鲁 、 王 红 妹 ） ， 卫 生 统 计 学 教 研 室 （ 沈 毅 ）SF-36健 康 调 查 量 表 中 文 版 的 研 制及 其 性 能 测 试李 鲁 王 红 妹 沈 毅【 摘 要 】 目 的 研 制 SF-36 健 康 调 查 量 表 中 文 版 并 验 证 量 表 维 度 建 立 及 记 分 假 设 、 信 度 和 效 度 。方 法 采 用 多 阶 段 混 合 型 等 概 率 抽 样 法 ， 用 SF-36 健 康 调 查 量 表 中 文 版 对 1 000 户 家 庭 的 居 民 进 行 自评 量 表 式 调 查 ； 参 照 国 际 生 命 质 量 评 价 项 目 的 标 准 程 序 ， 进 行 正 式 的 心 理 测 验 学 试 验 。 结 果 在 收 回的 1 985 份 问 卷 中 ， 18 岁 以 上 的 有 效 问 卷 1 972 份 ， 其 中 应 答 者 1 688 人 （ 85.6%） ， 1 316 人 回 答 了 所 有条 目 ， 372 人 有 1 个 或 以 上 的 缺 失 答 案 ， 无 应 答 者 中 文 盲 、 半 文 盲 占 65.5%。 等 距 假 设 在 活 力 （ VT） 和精 神 健 康 （ MH） 维 度 被 打 破 了 ， 按 重 编 码 后 值 计 算 维 度 分 数 ； 条 目 集 群 的 分 布 接 近 源 量 表 及 其 他 2 个中 文 译 本 ； 除 了 生 理 功 能 （ PF） 、 躯 体 疼 痛 （ BP） 、 社 会 功 能 （ SF） 维 度 ， 其 余 维 度 有 相 似 的 标 准 差 ； 除 了SF、 VT 维 度 ， 其 余 6 个 维 度 条 目 维 度 相 关 一 致 ； 除 了 SF 维 度 ， 7 个 维 度 集 合 效 度 成 功 率 范 围 为 75% 100%， 区 分 效 度 成 功 率 范 围 为 87.5% 100%。 一 致 性 信 度 系 数 除 了 SF、 VT 维 度 ， 其 余 6 维 度 变 化 范围 为 0.72 0.88， 满 足 群 组 比 较 的 要 求 。 两 周 重 测 信 度 变 化 范 围 为 0.66 0.94。 因 子 分 析 产 生 了 2个 主 成 分 ， 分 别 代 表 生 理 健 康 和 心 理 健 康 ， 解 释 了 56.3%的 总 方 差 。 结 论 为 SF-36 健 康 调 查 量 表 适用 于 中 国 提 供 了 证 据 ， 已 知 群 效 度 试 验 将 为 量 表 效 度 提 供 更 有 意 义 的 证 据 。【 关 键 词 】 生 活 质 量 ； 心 理 学 试 验 ； SF-36 量 表Development and psychometric tests of a Chinese version of the SF-36 Health Survey Scales LI Lu*，WANG Hongmei， SHEN Yi.*Department of Social Medicine， Zhejiang University School of Medicine， Hangzhou310031， China【 Abstract】 Objective To develop and evaluate scaling and scoring assumptions， and the reliability andthe validity of a Chinese version of the SF-36 scales. Methods A multi-stage mixed sampling procedure was usedto select a representative sample of the general population. The sample size was 1 000 households. All familymembers of a selected household， aged 18 and older， completed a survey by self-administration. Formalpsychometric methods for testing assumptions underlying item scoring and scale construction were used according tothe standard procedure of the IQOLA Project. Results Of the 1 985 collected questionnaires， 1 972 werequalified. Of them， 1 688（ 85.6%） were respondents. 1 316 respondents answered all 36 items， while theremaining（ 372 respondents） answered with one or several missing responses. Among the non-respondents， 65.5%were illiteracy or quasi-illiteracy. The assumption of equal intervals was violated for the VT and MH scales. Therecoded item values were used to calculate scale scores. The clustering and ordering of the item means wereapproximately the same as that of the source version and other two Chinese versions. The items in each scale hadsimilar standard deviations except those in the PF， BP， SF scales. The correlations between an item and itshypothesized scale were identical for all except the SF and VT scales. The scaling success rates of convergentvalidity were 0% for the SF scale， 75% for the VT scale， and 100% for the other six scales. The scaling successrates of discriminat validity ranged from 87.5% to 100% for all scales except for the SF scale. The Cronbachcoefficients of internal consistency reliability ranged from 0.72 to 0.88， which were satisfactory for groupcomparison except 0.39 for the SF scale and 0.66 for the VT scale. The two-weeks test-retest reliability coefficientranged from 0.66 to 0.94. Factor analysis identified two principal components： a“ physical” factor and a“ mental”factor. Taken together， these two factors could be used to explain 56.3% of the total variance.Conclusion TheChinese version of the SF-36 Health Survey Scale has achieved conceptual equivalence and satisfied thepsychometric scaling assumptions well enough to warrant wide use in China.
