陕 西 中 医 学 院硕 士 学 位 论 文清 肺 化 痰 逐 瘀 汤 对 AECOPD 痰 瘀 互 结 证 血 液 流 变 学 的 临 床 研 究姓 名 ： 郑 爱 红申 请 学 位 级 别 ： 硕 士专 业 ： 中 医 内 科 学指 导 教 师 ： 高 洁20100401摘 要目 的 ： 本 课 题 旨 在 通 过 清 肺 化 痰 逐 瘀 汤 对 AECOPD 痰 瘀 互 结 证 患 者 的 临 床 研 究 ，观 察 其 对 AECOPD 患 者 血 液 流 变 学 的 改 变 ， 评 价 清 肺 化 痰 逐 瘀 汤 对 AECOPD 患 者 的 治 疗作 用 ， 为 清 肺 化 痰 逐 瘀 汤 在 AECOPD 患 者 的 临 床 治 疗 提 供 依 据 。方 法 ： 选 取 符 合 入 选 标 准 的 慢 性 阻 塞 性 肺 疾 病 急 性 加 重 期 痰 瘀 互 结 证 患 者 60 例 ，随 机 分 为 2 组 ： 对 照 组 30 例 ， 给 予 控 制 性 吸 氧 ， 头 孢 他 啶 （ 对 头 孢 他 啶 过 敏 者 给 予头 孢 哌 酮 舒 巴 坦 ， 或 者 阿 奇 霉 素 ） ,配 合 左 氧 氟 沙 星 抗 感 染 ； 喘 息 者 给 予 氨 茶 碱 ,痰 稠不 易 咯 出 者 给 予 盐 酸 氨 溴 索 ； 治 疗 组 30 例 ， 给 以 口 服 清 肺 化 痰 逐 瘀 汤 ， 配 合 上 述 西医 治 疗 ； 观 察 两 组 患 者 临 床 症 状 、 血 、 尿 便 常 规 、 肝 、 肾 功 能 ， 胸 片 （ CT） 、 心 电 图 ，肺 功 能 等 一 般 安 全 性 检 测 及 血 液 流 变 学 的 改 变 。 2 周 为 1 疗 程 ， 观 察 一 个 疗 程 。 疗 程结 束 后 就 临 床 疗 效 和 安 全 性 作 出 评 价 。结 果 ： 在 对 两 组 患 者 进 行 性 别 、 年 龄 、 病 程 、 吸 烟 指 数 、 工 作 性 质 、 本 病 发 病 时长 等 进 行 统 计 学 处 理 ， 在 两 组 患 者 没 有 显 著 性 差 异 前 提 下 ， 经 加 用 清 肺 化 痰 逐 瘀 汤 治疗 ， 治 疗 后 两 组 症 状 比 较 ： 治 疗 组 咳 嗽 、 喘 息 、 舌 下 静 脉 迂 曲 有 明 显 差 异 （ P0.05） ，气短、颜面口唇紫绀、总积分有显著性差异（P 0.01） ， 说 明 治 疗 组 各 症 状 改 善 优 于对 照 组 。 血 液 流 变 学 分 析 ： 组 内 比 较 治 疗 组 病 例 治 疗 后 全 血 黏 度 、 血 浆 比 黏 度 、 纤 维蛋 白 原 定 量 有 显 著 性 差 异 （ P0.01） ， 红 细 胞 压 积 有 明 显 差 异 （ P0.05） ； 对 照 组 病 例治 疗 后 血 浆 比 黏 度 有 明 显 差 异 （ P0.05） ， 纤 维 蛋 白 原 定 量 有 显 著 差 异 （ P0.01） ；组间比较治疗后治疗组全血黏度和纤维蛋白原定量较对照组有显著差异（P 0.01） 。说明 治 疗 组 对 血 液 流 变 学 的 改 善 优 于 对 照 组 。结 论 ： 本 次 临 床 研 究 结 果 表 明 ， 清 肺 化 痰 逐 瘀 汤 可 以 明 显 减 轻 AECOPD 患 者 的 临床 症 状 ， 显 著 降 低 血 液 流 变 学 中 全 血 黏 度 、 血 浆 比 黏 度 、 纤 维 蛋 白 原 定 量 的 含 量 ， 明显 降 低 红 细 胞 压 积 。 在 服 药 过 程 中 ， 对 肝 肾 功 能 及 血 尿 常 规 无 明 显 影 响 ， 临 床 疗 效 显著 ， 无 明 显 不 良 反 应 ， 是 一 种 安 全 有 效 的 用 药 途 径 。关 键 词 ： AECOPD； 痰 瘀 互 结 证 ； 清 肺 化 痰 逐 瘀 汤 ； 血 液 流 变 学 ； 临 床 研 究The Blood rheology clinical research with Qing fei hua tan zhuyu decoction treat AECOPD phlegm stasis syndromeAbstractobjective: This topic is for the purpose of through tying the cardpatients clinical research mutually to the Qing fei hua tan zhu yu decoctionto the AECOPD phlegm stasis, observes it to the AECOPD patient blood rheologychange, appraises the Qing fei hua tan zhu yu decoction to the AECOPD patientstherapeutic action, patients clinical care provides the basis for the Qingfei hua tan zhu yu decoction in AECOPD.Methods: With the selection criteria for acute exacerbation of chronicobstructive pulmonary disease) and phlegm stasis syndrome, 60 randomlydivided into two groups: control group, 30 cases given oxygen, cefotaximes(cefotaximes allergic to give cefoperazone sulbactam was, or azithromycin),with levofloxacin anti-infection, Breathing, phlegm thick aminophyllineshall not give up the hydrochloric acid ammonia bromine cable, 30 cases intreatment group and give the Qing fei hua tan zhu yu decoction by westernmedicine therapy, cooperate with the soup, To observe the clinical symptoms,groups and routine blood and urine, liver, kidney, chest radiograph (CT),electrocardiogram, pulmonary function and so on security inspection andchange of blood rheology. 2 weeks for a course of treatment, observation.
