You must be 18 years of age or older to enroll.If you enroll through DMV, we will forward your information to the New York State Department of Health, which will include your name in the Registry and provide you an opportunity to limit your donation, if you choose to.What You Should Know if You Enroll in the Registry: Tell your family you have enrolled in the Registry and have given your legal consent for donation to occur upon your death. Your decision to donate will not affect your medical treatment. The organs and tissues you donate will go to patients in the greatest need. There is no cost to the donors family. You can have your name removed from the Registry at any time.
