Drivers ManualN E WY O R KS T A T ED E P A R T M E N TO FM O T O RV E H I C L E SMV-21 (2/11)Drivers ManualBecome an Organ and Tissue DonorEnroll in the New York State Donate LifeSMRegistry for Organ and Tissue Donations.To enroll in the Registry, read and sign the consent certification in the “Organ Donor” box on your Application For Driver License or Non-Driver ID Card (MV44) or where indicated on your application to Renew Your Driver License By Mail (MV-2L), or enroll anytime by contacting one of the organ donation organizations listed on this page. You must be 18 years of age or older to enroll.If you enroll through DMV, we will forward your information to the New York State Department of Health, which will include your name in the Registry and provide you an opportunity to limit your donation, if you choose to.What You Should Know if You Enroll in the Registry: Tell your family you have enrolled in the Registry and have given your legal consent for donation to occur upon your death. Your decision to donate will not affect your medical treatment. The organs and tissues you donate will go to patients in the greatest need. There is no cost to the donors family. You can have your name removed from the Registry at any time.
